What it actually does in the body

Carnosine is a dipeptide of beta-alanine and histidine, present in skeletal muscle at roughly 20 to 30 mmol/kg dry weight. Its imidazole ring has a pKa near 6.8 to 7.0, which places it almost perfectly for accepting protons across the exact pH range that muscle traverses during hard glycolytic work, from about 7.1 down toward 6.5. Evolution did not design it as a sports supplement, but it is very well positioned to act as one.

Carnosine synthase makes it, and the rate-limiting substrate is beta-alanine, not histidine. That single fact is the entire mechanism of the supplement: you are relieving the bottleneck on synthesis of an intramuscular buffer. Very few supplements have a mechanism this clean.

A correction while we are here, because the label gets it backwards. Lactate is not the acid burning your quads. The protons come from ATP hydrolysis during rapid glycolytic flux, and lactate production actually consumes a proton. Lactate is a fuel and a signalling molecule, not a metabolic villain. Carnosine buffers the H+ load, and it has secondary roles as a modulator of calcium sensitivity at the sarcoplasmic reticulum and as a scavenger of reactive carbonyl species.

Kinetics are the part athletes get wrong. Raising muscle carnosine takes weeks: 4 to 6 g/day for four weeks raises it roughly 40 to 60 percent, and ten weeks raises it further. It also washes out slowly, over weeks. There is therefore no reason at all to take beta-alanine before a workout. Consistency is the protocol; timing is irrelevant. Its presence in pre-workout blends is a marketing decision, and the tingle is the reason.

Which brings us to paresthesia. The tingling in your face and hands is beta-alanine acting on MrgprD receptors on cutaneous sensory neurons. It is harmless, dose-dependent, and eliminated by splitting doses to 1.6 g or less, or by using a sustained-release formulation. It is also, cynically, the most valuable ingredient in the pre-workout industry, because it makes a powder feel like it is working.

The tingle is a receptor, not a signal

Paresthesia is beta-alanine hitting MrgprD receptors in skin. It tells you the dose was large and taken at once. It tells you nothing about whether it is working.

What the literature supports

Hobson et al. 2012 meta-analysed around 40 studies and found the benefit concentrated in exercise lasting 60 to 240 s, with a median improvement in exercise capacity of roughly 2.9 percent and no significant effect below 60 seconds. Saunders et al. 2017 extended this with a larger set and agreed: clearest benefit from about 30 seconds to 10 minutes, strongest in the one-to-four-minute window.

The ISSN position stand (Trexler et al. 2015) concluded that 4 to 6 g/day for at least two to four weeks improves high-intensity exercise performance, with safety established at these doses out to 24 weeks. This is a supplement whose position stand and whose mechanism actually agree with each other.

Now the honesty the marketing skips. A single volleyball approach jump lasts under a second and is powered by phosphocreatine, not glycolysis. Beta-alanine will do nothing for it, and nothing for a shot put, a one-rep-max squat, or a 40-yard dash. Where it earns its place: long rallies and extended rally sets, repeated sprint blocks, the 400 and 800 metres, 2000 metre rowing, swim intervals, and high-rep resistance training taken near failure.

Marathon and other long endurance events fall outside the window too, from the other side. Buffering capacity is not what limits a two-hour effort.

Stacking: combining with sodium bicarbonate (an extracellular buffer) appears at least additive in some trials, since the two act in different compartments. Combining with creatine addresses two different energy systems and has no clear interaction beyond that.

The window is the verdict

Roughly 2.9 percent in efforts lasting one to four minutes. Nothing measurable under a minute. If your sport is jumps, save your money.

Supported, with a specificity that most supplements never earn. The mechanism is clean, the rate-limiting step is known, the effect window is defined by that mechanism, and the meta-analyses land inside the window the mechanism predicts. That coherence is rare and it is why this sits in the top tier alongside creatine.

It is also a supplement most people buy for the wrong sport. If your event is a series of one-second explosions, beta-alanine has nothing for you. If you spend two minutes at a time trying not to slow down, it has about three percent, which at the top of a season is a real number.

Who this is actually for

Athletes with sustained high-intensity efforts of roughly one to four minutes: 400 and 800 metres, 2 km rowing, swim intervals, repeated sprint sports with short rest, and high-rep resistance work near failure. 4 to 6 g/day, split, every day, for at least four weeks.

Who should not bother

Pure power athletes (jumpers, throwers, powerlifters, sprinters under 30 seconds) and long endurance athletes. Both sit outside the window, at opposite ends.

Safety and interactions

  • Paresthesia (harmless tingling) is dose-dependent. Split doses to 1.6 g or less, or use sustained release.

  • Chronic beta-alanine may reduce taurine uptake through a shared transporter. This has been shown in rodents and has not been demonstrated as a problem in humans.

  • No data in pregnancy or in children. That absence is a reason not to, not a reassurance.

  • Buy third-party tested (Informed Sport, NSF Certified for Sport), especially if you are subject to drug testing.
